6 C 2-401.11: (A) Except as specified in ¶ (B) of this section, an EMPLOYEE shall eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed FOOD; clean EQUIPMENT, UTENSILS, and LINENS; unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES; or other items needing protection can not result. (B) A FOOD EMPLOYEE may drink from a closed BEVERAGE container if the container is handled to prevent contamination of: (1) The EMPLOYEE'S hands; (2) The container; and (3) Exposed FOOD; clean EQUIPMENT, UTENSILS, and LINENS; and unwrapped SINGLE-SERVICE and SINGLE-USE ARTICLES.
Observed employee drink stored by lemonade and packaged sugar at bottom shelf below tea dispenser. Remove these drinks before the nex routine inspection.
16 P 4-501.114 (A):  (A) A chemical SANITIZER used in a SANITIZING solution for a manual or mechanical operation at contact times specified under ¶4-703.11(C) shall meet the criteria specified under §7-204.11 Sanitizers, Criteria, shall be used in accordance with the EPA- registered label use instructions, and shall be used as follows: (A) A chlorine solution shall have a minimum temperature based on the concentration and pH of the solution as listed in the following chart in the Food Code.
Observed sanitizer was too weak at dishwasher. Sanitizer was adjusted by employee. COS
21 P 3-501.16(A)(1): (A) Except during preparation, cooking, or cooling, or when time is used as the public health control as specified under §3-501.19, and except as specified under ¶ (B) and in ¶ (C ) of this section, TIME/TEMPERATURE CONTROL FOR SAFETY FOOD shall be maintained: (1)At 57°C (135°F) or above, except that roasts cooked to a temperature and for a time specified in ¶ 3-401.11(B) or reheated as specified in ¶ 3-403.11(E) may be held at a temperature of 54°C (130°F) or above.
Observed mashed potatoes were at 75F. Cook reheated mashed potatoes to 165 F and or above. COS
39 C 3-305.11 (A) (B) (C): (A) Except as specified in ¶¶ (B) and (C) of this section, FOOD shall be protected from contamination by storing the FOOD: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 15 cm (6 inches) above the floor. (B) FOOD in packages and working containers may be stored less than 15 cm (6 inches) above the floor on case lot handling EQUIPMENT as specified under § 4-204.122. (C) Pressurized BEVERAGE containers, cased FOOD in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture.
Observed no lid on tomatoes, cheese etc in pizza reach-in cooler. Provide lid or plastic on these foods before the next routine inspection.
41 C 3-304.14 (B)(1)(2): (B) Cloths in-use for wiping counters and other EQUIPMENT surfaces shall be: (1) Held between uses in a chemical sanitizer solution at a concentration specified under § 4-501.114; and (2) Laundered daily as specified under ¶ 4-802.11(D).
Observed soiled wiping cloth left out. Store in sanitizer bucket when not in use before the next routine inspection.
43 C 3-304.12 (C): During pauses in FOOD preparation or dispensing, FOOD preparation and dispensing UTENSILS shall be stored: (C) On a clean portion of the FOOD preparation table or cooking EQUIPMENT only if the in-use UTENSIL and the FOOD- CONTACT surface of the FOOD preparation table or cooking EQUIPMENT are cleaned and SANITIZED at a frequency specified under §§ 4-602.11 and 4-702.11.
Observed knives were store between prep table and reach-in cooler. This was removed. COS
44 C 4-904.11 (A): (A) SINGLE-SERVICE and SINGLE-USE ARTICLES and cleaned and SANITIZED UTENSILS shall be handled, displayed, and dispensed so that contamination of FOOD- and lip-contact surfaces is prevented.
Observed forks and spoons were facing upward in container. Store them so one can pick them up by handles only before the next inspection.
45 C 4-903.11 (C): (C) SINGLE-SERVICE and SINGLE-USE ARTICLES shall be stored as specified under ¶ (A) of this section and shall be kept in the original protective PACKAGE or stored by using other means that afford protection from contamination until used.
Observed box's of carry out bags were stored on floor by back dry storage. Store 6 inches off the floor before the next routine inspection. Repeat
47 C 4-501.11 (A): (A) EQUIPMENT shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 and 4-2.
Observed seal gaskets are torn on most reach-in cooler drawers. Replace these seal gaskets before the next routine inspection.
49 C 4-601.11(C): (C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of dust, dirt, FOOD residue, and other debris.
Observed the following non-food contact equipment needs cleaned: 1. Bottom of prep table has a lot of debris on it below tea dispensers. 2. Top of dishwasher has debris on it. 3. Grease receptor has a lot of grease and debris on it under back prep sink. 4. Interior of pizza machine has debris on it. 5. Bottom of True freezer has food debris on it. 6. Interior of fryer cabinets have grease and food debris in them. 7. Sides of fryers have grease and debris on them. 8. Seal gaskets have food debris in them. Repeat
54 C 5-501.115: A storage area and enclosure for REFUSE, recyclables, or returnables shall be maintained free of unnecessary items, as specified under § 6-501.114, and clean.
Observed empty boxes found in back located by maintenance room. Remove all empty boxes before the next inspection.
54 C 5-501.113: Receptacles and waste handling units for REFUSE, recyclables, and returnables shall be kept covered: (A)Inside the FOOD ESTABLISHMENT if the receptacles and units: (1)Contain FOOD residue and are not in continuous use; or (2)After they are filled; and (B)With tight-fitting lids or doors if kept outside the FOOD ESTABLISHMENT.
Observed dumpster lid was left open. Keep shut when not in use before the next routine inspection.
55 C 6-201.12: (A) Utility service lines and pipes may not be unnecessarily exposed. (B) Exposed utility service lines and pipes shall be installed so they do not obstruct or prevent cleaning of the floors, walls, or ceilings. (C) Exposed horizontal utility service lines and pipes may not be installed on the floor.
Observed the following floors, walls and ceilings need to be cleaned: 1. Wall under the three compartment sink has debris on it. 2. Floor under tea pots has debris on it. 3. Floor under dishwasher has debris on it. 4. Water on floor in front of dressing reach-in cooler. 5. Wall by ice machine has dust on it. 6. Ceiling tiles in kitchen have dust and debris on them. 7. Mop sink has debris in it. 8. Vents have dust on them. Grill hood filters have grease and debris on them. 9. Floor under prep sink has debris on it. 10. Wall by Deli meats in walk-in cooler has mold growth on it. 11. Floor in walk-in cooler ad freezer has food debris on it. 12. Floor under chicken reach-i cooler has debris on it. 13. Floor under fryers and flat stove has a lot of grease and food debris on it. 14. Wall above bottled sauces have sauces splattered on it. 15. Floor drains in bar have debris around and in them. 16. Wall in keg walk-in cooler has mold on it. 17. Ceiling in Keg walk-in cooler has mold growth on it. Clean and maintain walls, ceilings and floor before the next routine inspection. Repeat
